Description
Ventricular system anatomy is centered on the paired lateral ventricles, their anterior (frontal) horns extending anteroinferiorly, bodies lying deep to the corpus callosum, and posterior (occipital) and inferior (temporal) horns sweeping posterolaterally and inferolaterally within the cerebral hemispheres. Medially, each lateral ventricle communicates with the third ventricle through the interventricular foramen (foramen of Monro), bordered by the columns of the fornix anteriorly and the thalamus posteriorly. The third ventricle sits in the midline between the thalami, narrowing posteriorly into the cerebral aqueduct within the midbrain and opening into the fourth ventricle, which lies dorsal to the pons and medulla and ventral to the cerebellum. Choroid plexus contours are indicated along the ventricular roofs where cerebrospinal fluid is produced. Ventricular size and configuration are daily decision points in neuroradiology and neurosurgery, and this illustration supports that pattern recognition by keeping the CSF spaces legible against surrounding parenchymal landmarks. Aqueductal stenosis, obstruction at the foramina of Monro, or impaired CSF egress from the fourth ventricle to the subarachnoid space (via the median aperture of Magendie and lateral apertures of Luschka) produces characteristic upstream dilatation that clinicians describe as noncommunicating hydrocephalus. Orientation matters. Being able to relate the temporal horn to the hippocampal formation and the third ventricle to the thalami helps when correlating headache, gait disturbance, papilledema, or altered mental status with CT or MRI findings.
