Anterior quadrangular lobule (lobulus quadrangularis anterior) occupies the superolateral aspect of the anterior cerebellar lobe in this anterior-facing perspective. Its folia sweep laterally across the superior surface of the cerebellar hemisphere, lying lateral to the superior vermis and superior to the anterior margin that approaches the brainstem. The primary fissure forms the posterior boundary of the anterior lobe, separating this lobule from more posterior hemispheric territory, while the midline vermian components remain more medial and compact. Fissures and folial patterning are emphasized on the superior-anterior surface. Functionally, the anterior lobe is classically linked to the spinocerebellum, so lesions involving the anterior quadrangular lobule and adjacent anterior lobe cortex often present with ipsilateral limb ataxia, dysmetria on finger-to-nose testing, and gait instability that clinicians may localize to a cerebellar hemisphere rather than a purely vermian syndrome. The anterior viewpoint supports teaching of lobar boundaries because the superior surface and the relationship to the primary fissure are readable without the visual clutter of posterior lobules, making it easier to correlate surface anatomy with regional infarcts in the superior cerebellar artery territory and with compressive effects from tentorial or posterior fossa masses.
