Description
Dominating the anterior mediastinum, the heart is shown in situ with its sternocostal surface formed largely by the right ventricle and the left ventricle contributing the left margin and apex. Superiorly, the ascending aorta rises from the left ventricle and continues into the aortic arch, while the pulmonary trunk emerges anterior to the aorta before dividing toward the pulmonary arteries. Venous return is oriented on the right: the superior vena cava enters the right atrium superiorly, and the inferior vena cava approaches the right atrium inferiorly. Ribcage landmarks frame the thorax, with the sternum anterior to the pericardial silhouette and the lungs implied laterally. For teaching and clinical planning, the anterior surface matters because it is the face of the heart encountered in median sternotomy and pericardiocentesis considerations, and it explains why right ventricular structures dominate the operative field once the sternum is divided. Coronary distribution is often taught from this aspect, since the anterior interventricular territory corresponds to the left anterior descending artery and becomes the classic ECG-infarct correlation in anterior wall STEMI. Orientation is everything.
