Description
Seen from a posterior perspective in the adult male, the ascending aorta curves into the aortic arch and continues as the descending thoracic aorta as it tracks inferiorly just left of the vertebral bodies. Superiorly, the arch gives rise to the brachiocephalic trunk, left common carotid artery, and left subclavian artery, while the pulmonary trunk and proximal pulmonary arteries lie more anterior, and the superior vena cava sits to the right of the ascending aorta. On the cardiac base, the left atrium is positioned posteriorly, and the coronary arteries course over the epicardial surface, with the posterior interventricular sulcus marking the route of the posterior descending artery and its companion veins. Posterior aortic anatomy matters when you need to orient to the mediastinum from the back, where the descending thoracic aorta is a common reference in cross sectional imaging and in posterior approaches to the thoracic spine. Aneurysm, dissection, and traumatic aortic injury most often involve the isthmus region distal to the left subclavian artery, and this angle clarifies how close that segment runs to the left paravertebral gutter and esophagus. It also supports teaching of why aortic pathology can present with interscapular pain and why a left posterolateral thoracotomy targets the descending thoracic aorta.
