Description
Running in a central sagittal section through the male heart, the ascending aorta rises superiorly from the left ventricle at the left ventricular outflow tract, immediately posterior to the sternocostal surface and just anterior to the left atrium. At its base, the aortic root is continuous with the aortic valve, and the thickness of the adjacent interventricular septum and left ventricular free wall is apparent when compared with the thinner-walled right ventricle located more anteriorly. Portions of the mitral and tricuspid valves are seen in profile, with chordae tendineae and papillary muscle attachments implied by the cut, while the aortic arch curves superiorly and posteriorly as it exits the pericardial silhouette. Wall architecture reads clearly. This sagittal relationship between the aorta and the left ventricle is the teaching sweet spot for understanding afterload and why left ventricular hypertrophy accompanies long-standing systemic hypertension or aortic stenosis. The cut also clarifies how pathology at the aortic root, such as dilation at the sinuses of Valsalva or a Type A aortic dissection, can compromise the aortic valve and rapidly produce acute aortic regurgitation with left ventricular volume overload. Orientation matters for surgeons and imagers.
