Description
Cut through the right half of the encephalon in sagittal section, the cerebral hemispheric cortex and underlying white matter sit superior to the brainstem, while the cerebellum occupies the posteroinferior fossa behind the pons and medulla. A lenticulostriate artery is picked out as a fine perforating branch coursing superiorly and medially from the proximal middle cerebral artery region toward the deep gray, supplying the basal ganglia and adjacent internal capsule territory. In the neck, yellow cranial nerves descend from the medulla and upper cervical cord, and the carotid and jugular vessels run anterolaterally in parallel, reinforcing the tight neurovascular packing of the carotid space. Small vessel. Big consequences. Lenticulostriate perforators are the classic substrate for hypertensive lipohyalinosis and Charcot-Bouchard microaneurysms, the usual culprit in primary deep intracerebral hemorrhage involving the putamen and posterior limb of the internal capsule. This lateral sagittal cut helps you relate a tiny arteriole to disproportionate deficits, pure motor hemiparesis or dysarthria-clumsy hand syndrome, by placing its end-artery supply adjacent to corticospinal fibers.
