Running through the midsagittal section, the anterior cerebral artery (ACA) is traced as it courses anterosuperiorly from the region of the internal carotid termination, curves around the rostrum and genu of the corpus callosum, and continues posteriorly along the pericallosal sulcus on the medial surface of the cerebral hemisphere. Inferior to its arch lie the lamina terminalis and optic chiasm region, while the cingulate gyrus sits immediately superior to the callosal body, separated by the callosal sulcus. Posteriorly, the cerebellar vermis and fourth ventricle align behind the brainstem, and the midbrain, pons, and medulla oblongata stack superior to inferior as the spinal cord descends through the foramen magnum. Clear midline landmarks. That callosomarginal and pericallosal relationship is the teaching point you want when discussing medial frontal and parietal perfusion, because ACA territory strokes characteristically produce contralateral leg-predominant weakness and sensory loss from involvement of the paracentral lobule, often with abulia when the medial frontal cortex and cingulate are affected. Aneurysms at the anterior communicating artery complex, just anterior to the lamina terminalis and near the optic apparatus, also make sense in this orientation, and the sagittal cut helps explain why rupture can spill into the interhemispheric fissure and ventricular system.
