Description
Shown in lateral aspect after a sagittal hemisection, the cerebral hemisphere is opened to reveal the medial cortical surface with the corpus callosum arching superior to the lateral ventricle and diencephalon. Inferior and posterior to the thalamus, the midbrain continues caudally into the pons and medulla, with the cerebellum positioned posteroinferior to the brainstem. The superior cerebellar artery arises near the basilar bifurcation and courses posteriorly, wrapping around the lateral midbrain in the ambient cistern before distributing to the superior (tentorial) surface of the cerebellar hemispheres and vermis. Its relationship to cranial nerve III at the level of the interpeduncular fossa is clear. This lateral sagittal presentation is a clean way to teach posterior circulation anatomy because it preserves the spatial relationship between the basilar apex, posterior cerebral artery origin, and the superior cerebellar artery as it encircles the midbrain. Neurosurgeons and neurointerventionalists watch this vessel closely in basilar tip aneurysms, where clipping or coiling can threaten superior cerebellar artery patency and produce cerebellar infarction with limb ataxia and dysmetria. Compression or distortion of the oculomotor nerve can accompany pathology in this cisternal region. Small territory, big consequences.
