Description
Viewed from a superior vantage into the opened cranial cavity, the basilar artery ascends along the clivus and gives rise to the anterior inferior cerebellar artery (AICA) from its lower to mid basilar trunk. Each AICA courses laterally and slightly posteriorly toward the cerebellopontine angle, running inferior to the posterior cerebral circulation and in close neighborhood to the vertebrobasilar junction and the proximal posterior inferior cerebellar artery territory. More rostrally, the Circle of Willis is suggested by the internal carotid arteries with anterior and middle cerebral branches, and by the posterior communicating and posterior cerebral arteries at the basilar apex. Small blue channels imply venous drainage along the dural reflections. AICA anatomy matters because its perforators and cortical branches supply the anterior inferior cerebellum and portions of the lateral pons, territories implicated in AICA infarction with vertigo, ataxia, facial weakness, and hearing symptoms when labyrinthine artery supply is involved. This arterial segment also sits on the operative corridor for cerebellopontine angle procedures and microvascular decompression, where arterial loops near the internal auditory canal and cranial nerves VII and VIII can become surgical landmarks or sources of neurovascular compression. Variants are common. AICA may be hypoplastic, duplicated, or functionally replaced by an enlarged PICA. Use this illustration for teaching vertebrobasilar circulation in neuroanatomy and head and neck gross anatomy, or for stroke-focused lectures that contrast PICA, AICA, and SCA syndromes at the brainstem and cerebellum.
