Description
Cut away through the calvaria, the cranial cavity is opened to trace the anterior inferior cerebellar artery (AICA) as it courses from the basilar artery along the ventral surface of the cerebellum toward the cerebellopontine angle. From this oblique cranial perspective, the AICA runs inferior to the pons and anterior to the cerebellar hemisphere, with its looping segment approaching the internal acoustic meatus at the petrous part of the temporal bone. Surrounding osseous landmarks such as the orbits, nasal cavity, maxillary dentition, and the cut margins of the skull help orient the vessel’s position within the posterior cranial fossa. Red arterial coding isolates the AICA within the broader intracranial arterial network. AICA anatomy matters because its territory overlaps structures that drive common otoneurologic presentations. The labyrinthine artery often arises from the AICA, so ischemia or vasospasm can present with sudden sensorineural hearing loss and vertigo, and the vessel’s proximity to cranial nerves VII and VIII makes it a familiar offender in vascular loops associated with neurovascular compression syndromes.
