Superior Perspective of the Anterior Choroidal Artery in the Male Skull

Removed calvaria reveals the superior surface of the male cranial cavity with the cerebral arterial tree highlighted, centered on the Circle of Willis around the optic chiasm and infundibular region. From the internal carotid artery, the anterior cerebral artery courses anteromedially toward the interhemispheric fissure, while the middle cerebral artery runs laterally into the Sylvian fissure; posteriorly, the posterior cerebral arteries arise from the basilar bifurcation and sweep posterolaterally around the midbrain. The anterior choroidal artery is seen as a slender branch from the supraclinoid internal carotid, passing posteriorly and slightly laterally toward the choroidal fissure and the temporal horn, deep to the larger proximal trunks. That small caliber belies its territory: the anterior choroidal artery supplies the posterior limb of the internal capsule, optic tract and lateral geniculate body, globus pallidus, and the choroid plexus of the lateral ventricle. Infarction from internal carotid atherosclerosis, embolus, or iatrogenic compromise during aneurysm clipping or endovascular work near the posterior communicating segment classically produces contralateral hemiparesis, hemisensory loss, and homonymous hemianopia. Use this superior perspective to teach neurovascular anatomy in gross anatomy, neuroradiology correlation (CTA, MRA, DSA), and neurosurgical planning modules that emphasize the relationship of the skull base to supraclinoid internal carotid branches.
