Description
Seen from a lateral perspective, the lower lumbar spine and pelvis frame the proximal femur and hip joint, with the femoral head seated in the acetabulum and the femoral neck projecting anteromedially toward the shaft. Lumbar spinal nerves contribute to the lumbar plexus within and along the posterior abdominal wall, and the femoral nerve then courses inferiorly toward the anterior thigh deep to the inguinal region. A fine articular branch departs the femoral nerve proximally and tracks toward the anterolateral hip capsule, running anterior to the hip joint axis and close to the iliopsoas compartment. Small, specific anatomy. Clinically, the articular branch helps explain anterior hip pain patterns and why hip pathology can refer discomfort to the anterior thigh, even when motor testing of quadriceps strength appears intact. This trajectory also matters during anterior and anterolateral approaches to the hip and during pericapsular nerve group (PENG) blocks, where local anesthetic targets articular innervation from the femoral nerve (and nearby obturator contributions) along the iliopubic eminence and superior pubic ramus. Denervation strategies for hip osteoarthritis and post arthroplasty pain management often discuss these branches explicitly, so having the nerve’s relationship to the hip capsule and proximal femur clarified in a single lateral view saves time when teaching or planning.
