Description
Arising from the lumbar plexus within the psoas major, the femoral nerve descends between psoas major and iliacus and passes deep to the inguinal ligament into the femoral triangle. Anteriorly, its trunk lies lateral to the femoral artery and outside the femoral sheath, then divides quickly into anterior and posterior divisions that fan out across the proximal thigh. Cutaneous branches track superficially toward the anteromedial thigh, while muscular branches run deep toward iliopsoas and the quadriceps femoris group. Clear separation. For clinicians, this anterior exposure is the one you want when teaching a femoral nerve block at the inguinal crease, where the nerve sits just lateral to the femoral artery and deep to fascia iliaca. The branching pattern in the femoral triangle also explains common pitfalls: anesthetic deposited too medially risks vascular puncture, while too superficial an injection can spare motor branches to vastus medialis and rectus femoris. Traumatic or iatrogenic femoral neuropathy, after pelvic surgery or retroperitoneal hematoma, typically presents with weak knee extension and reduced patellar reflex, and this illustration ties those deficits to the nerve’s course across iliacus and beneath the inguinal ligament.
