Description
Posterior to the auricle, the elastic auricular cartilage forms the framework of the pinna as it lies lateral to the mastoid process of the temporal bone and inferior to the lambdoid suture region of the occiput. The cranial sutures (sagittal, lambdoid, and portions of the coronal) sit superior and medial to the external ear, while the cervical spine aligns in the midline deep to the nuchal soft tissues. Mastoid anatomy anchors the ear posteriorly, and the posterior attachments of auricular muscles blend into the fascia overlying the occipital and suboccipital musculature. Small emissary venous channels are emphasized deep to the surface, coursing through the mastoid region toward the dural venous sinuses. Clinical relevance concentrates in that mastoid and retroauricular corridor. Emissary veins can provide a pathway for infection to spread between scalp or mastoid air cells and intracranial venous sinuses, a classic concern in mastoiditis with potential septic thrombosis of the sigmoid sinus. This posterior perspective also matches the working field for a postauricular approach in otologic surgery, where elevation of the auricle exposes the mastoid cortex and guides safe dissection relative to the sigmoid sinus and posterior fossa boundaries. Bleeding here is usually low pressure, but it can be persistent and misleading. Otology and head and neck anatomy courses use this plate to teach surface-to-deep relationships around the pinna, mastoid process, and cervical midline, and it pairs well with lectures on venous drainage and routes of infection. It also supports surgical atlases, ENT patient education materials, and radiology correlation when explaining why retroauricular swelling, mastoid tenderness, or intracranial complications can be linked.
