Description
Obliquely from the left thorax, the left brachiocephalic vein is traced as it forms behind the left sternoclavicular joint from the union of the left subclavian vein and left internal jugular vein, then courses medially and slightly inferiorly across the superior mediastinum. Anterior to the great arterial trunks, it passes toward the right to join the right brachiocephalic vein and create the superior vena cava, which descends to the right atrium. Adjacent landmarks include the aortic arch and its branches (brachiocephalic trunk, left common carotid artery, left subclavian artery), the pulmonary trunk, and surface coronary vessels on the heart. Bony references from the anterior ribs and posterior vertebral column anchor the mediastinal depth. Left-sided renderings clarify a common point of confusion in teaching: the left brachiocephalic vein is longer and more transverse than its right counterpart, and its anterior relationship to the aortic arch branches explains why it can be vulnerable during median sternotomy, thymic surgery, or reoperative cardiac procedures. Line and device placement benefits from this anatomy too; left internal jugular or subclavian central venous catheterization must negotiate the venous angle and the long oblique course before entering the superior vena cava, and malposition can occur if the catheter tracks into smaller tributaries. A short vein. A long crossing. That contrast matters.
