Description
Arising along the obtuse (left) margin of the heart, the left marginal vein courses inferiorly on the lateral wall of the left ventricle and tracks toward the coronary sulcus to join the great cardiac vein before its continuation into the coronary sinus. The left atrium sits superior and posterior to this pathway, while the left ventricle forms the dominant anterior and inferior contour, with the left auricle projecting anterosuperiorly near the pulmonary trunk. Epicardial fat and the superficial coronary vessels outline the atrioventricular groove, and the aorta and pulmonary arteries anchor the superior aspect of the specimen. Venules on the ventricular surface converge into the marginal vessel as it approaches the base. This left-sided surface view matters because the marginal venous channel runs close to the left ventricular free wall where surgeons and electrophysiologists commonly work, and it helps you orient the coronary sinus drainage pattern relative to the great cardiac vein. Venous anatomy here is clinically relevant during cardiac resynchronization therapy and other coronary sinus based procedures, where variant venous caliber or absence of a suitable branch can limit left ventricular lead placement or increase the risk of venous dissection. Small structure, real consequences. Use this asset in gross anatomy and cardiothoracic modules to teach coronary venous drainage alongside the coronary arteries, and in interventional cardiology or EP education when explaining coronary sinus cannulation landmarks and lateral left ventricular targeting. It also fits well in atlases, review articles, and device training materials that need a clean depiction of the left marginal vein in relation to the left ventricle and adjacent great vessels.
