Description
Seen from the left lateral aspect, the left ventricle dominates the field, tapering inferiorly to the apex while the left atrium sits superior and posterior at the base. Along the diaphragmatic (inferior) surface of the left ventricle, the inferior vein of the left ventricle courses in the epicardial fat toward the coronary sinus, running roughly parallel to adjacent coronary arterial branches over the myocardium. The aorta emerges superiorly from the left ventricular outflow tract, and the pulmonary trunk with proximal pulmonary arteries lies anterior and slightly superior in relation to the left atrium. For coronary venous anatomy, this is the angle you want. The inferior vein of the left ventricle is a frequent tributary of the coronary sinus and becomes clinically relevant during cannulation of the coronary sinus for retrograde cardioplegia, placement of left ventricular pacing leads via the coronary venous system, and electrophysiology mapping when venous landmarks help orient the posterolateral left ventricular wall. Small venous tributaries vary, and appreciating their surface course helps avoid misidentifying a vein as a coronary artery during operative exposure of the atrioventricular groove. Use this rendering in gross anatomy and cardiovascular modules when teaching the external topography of the left ventricle and its epicardial vessels, or in cardiothoracic surgery and cardiology publications discussing coronary sinus access, cardiac resynchronization therapy lead routes, or venous variants on the inferior left ventricular surface. It also supports patient education graphics where arterial (red) versus venous (blue) pathways must remain visually distinct without opening the chambers.
