Description
Presented from an inferior aspect, the left ventricle dominates the field, with the diaphragmatic surface of the myocardium curving toward the cardiac apex. The inferior vein of the left ventricle courses superficially within the epicardial fat, trending obliquely toward the posterior atrioventricular (coronary) sulcus where it approaches venous confluence with the coronary sinus region. Superior to the ventricular mass, the left atrium is partially inferred at the base, and segments of the great vessels are cut in cross section at their proximal openings. That venous channel matters because it helps define the posterior coronary venous anatomy encountered in electrophysiology and cardiac surgery. During cannulation for retrograde cardioplegia via the coronary sinus, variation in left ventricular venous drainage can complicate catheter positioning and influence perfusion of the inferolateral wall. Ischemia here is common. Use this asset in cardiothoracic anatomy teaching to contrast inferior versus anterior coronary vessel patterns, or in a surgical atlas discussing coronary sinus cannulation, mitral valve exposure from the diaphragmatic surface, and mapping targets for ventricular arrhythmia ablation along the inferobasal left ventricle.
