Description
Caudal spinal trigeminal nucleus (nucleus spinalis nervi trigemini, pars caudalis) forms a longitudinal, vertically oriented cell column along the dorsolateral medulla in this lateral view of the brainstem. Its gray matter lies just medial to the spinal trigeminal tract (tractus spinalis nervi trigemini) and posterior to the ventrolateral medullary surface, with the inferior olive positioned more anteromedially. Superiorly, the column continues toward the interpolar and oral parts of the spinal trigeminal complex, while inferiorly it blends with the upper cervical dorsal horn at the cervicomedullary junction. Boundaries read cleanly. Clinically, this is the medullary relay for pain and temperature from the ipsilateral face, cornea, oral cavity, and meninges, receiving primary afferents from CN V and also contributions from CN VII, IX, and X. Infarction in the lateral medulla (Wallenberg syndrome), often from PICA territory ischemia, commonly injures this nucleus and tract, producing loss of facial pain and temperature with the classic “crossed” pattern when the adjacent anterolateral system is also involved. The fixed lateral perspective helps you localize the nucleus relative to the ventrolateral medullary landmarks used in brainstem lesion correlation and in teaching brainstem vascular syndromes. Neuroanatomy and neurology curricula can pair this illustration with sensory pathway schematics to anchor the trigeminal nociceptive system to a real medullary location, and neuropathology or stroke atlases can use it to support lateral medullary syndrome localization tables.
