Description
Dorsal horn grey matter is highlighted as an elongated dorsolateral column within the medulla oblongata in a fixed lateral (side) perspective. Its position sits posterior to the ventrolateral medullary surface and lateral to the midline pyramidal region, aligning with the caudal brainstem where spinal cord grey matter transitions into brainstem sensory nuclei. Along the lateral medullary contour, the column lies deep to the external surface and near the expected course of the spinal trigeminal tract and the region that continues rostrally toward the pons. Orientation is anchored by the long axis of the brainstem, with the highlighted grey matter extending superior to inferior in parallel with the medullary neuraxis. Clinically, this dorsolateral medullary grey matter is most often discussed in relation to pain and temperature pathways of the face and head, because it corresponds topographically to the spinal trigeminal nucleus and its adjacent tract in the caudal medulla. Small posterior inferior cerebellar artery territory infarcts that produce lateral medullary (Wallenberg) syndrome commonly involve this zone, explaining ipsilateral loss of facial pain and temperature while sparing dorsal column modalities. The side perspective helps when teaching lesion localization since it keeps the dorsoventral relationships intuitive, placing the sensory nuclei posterior to corticospinal structures without forcing a cross-sectional mental flip.
