Description
Caudal pontine reticular nucleus (nucleus reticularis pontis caudalis) occupies the caudal pontine tegmentum as an elongated column within the reticular formation. In a lateral brainstem profile, it sits dorsal to the basilar part of the pons and ventral to the floor of the fourth ventricle, extending superior-inferiorly toward the pontomedullary junction. Its position is medial to the middle cerebellar peduncle and posterior to the corticospinal and corticobulbar bundles traversing the ventral pons, anchoring it within the core of the brainstem rather than at the surface. Borders matter here. Functionally, the caudal pontine reticular nucleus is a major relay in the pontine reticulospinal system, integrating auditory and somatosensory input into rapid motor output, and it is repeatedly cited in work on the acoustic startle reflex and pontine gating of arousal. Lesions from basilar artery perforator infarcts or pontine hemorrhage that involve the paramedian tegmentum can disrupt reticular activating networks, contributing to impaired consciousness, abnormal startle, and disordered sleep-wake regulation.
