Description
Anterior thoracic wall anatomy is presented on an adult obese male, with the pectoral region filling the frame from the clavicular level superiorly to the upper abdominal contour inferiorly. Bilateral nipples and areolae mark the superficial landmarks of the mammary region, positioned symmetrically on either side of the midline overlying the pectoralis major and the underlying costal cartilages. Subcutaneous adipose tissue thickens the anterior chest contour, softening the definition of the sternum and intercostal spaces while maintaining left to right symmetry across the parasternal and midclavicular regions. Normal surface findings, including scattered nevi, freckles, and chest hair, are visible against an even, neutral background. Obesity changes what you can and cannot trust on surface exam of the thorax, and this anterior view makes that point cleanly. Increased adiposity can obscure sternal borders and rib margins, complicating palpation for sternal tenderness after blunt trauma, counting intercostal spaces for pleural procedures, and assessing chest wall asymmetry. It also intersects with male breast and chest wall evaluation: pseudogynecomastia from adipose deposition can mimic true glandular gynecomastia, and nipple position may appear more inferior relative to the inframammary fold, influencing clinical documentation and preoperative planning.
