Description
Arising from the inferior surface of the subclavian artery, the internal thoracic artery descends vertically along the inner aspect of the anterior thoracic wall, just lateral to the sternum and deep to the costal cartilages. Its course is framed by the clavicle superiorly and the rib cage anterolaterally, with the vessel running parallel to the sternal margin toward the superior epigastric continuation inferiorly. Surrounding anatomy in the upper male torso logically includes the manubrium and body of the sternum, anterior ribs and costal cartilages, and adjacent venous companions (internal thoracic veins), with regional nerves of the lower cervical and upper thoracic segments crossing the field. Orientation is clear. Relationships matter. For cardiothoracic work, this artery is the standard conduit for coronary artery bypass grafting, most often anastomosed to the left anterior descending artery, so a clean depiction of its origin and parasternal descent helps you teach safe harvest planes and common anatomical variants. Proximity to the pleura and anterior intercostal spaces explains why pleural breach, bleeding from internal thoracic branches, or postoperative parasternal pain can occur when dissection strays laterally. The parasternal position also anchors clinical procedures, including anterior chest wall flap perfusion planning and the operative corridor near the sternum during median sternotomy. Gross anatomy and surgical anatomy courses use this view to link surface landmarks (sternal border, intercostal spaces) with the internal thoracic vascular pathway, and it drops neatly into CABG atlases, thoracic surgery chapters, or interventional radiology teaching on anterior chest wall bleeding and embolization targets. It also suits patient-facing diagrams when explaining graft selection and why the left internal thoracic artery is favored for long-term patency.
