Description
Centered along the parasternal chest wall, the paired internal thoracic veins ascend on either side of the sternum, running superiorly just posterior to the costal cartilages and medial to the anterior intercostal spaces. Each vein parallels the internal thoracic artery, receiving anterior intercostal veins segmentally and small tributaries from the pectoral and mediastinal region before converging superiorly. Near the thoracic inlet, the internal thoracic vein drains into the ipsilateral brachiocephalic vein at the junction where the subclavian vein and internal jugular vein unite. Close relationships to the clavicle, first rib, and manubrium provide bony landmarks for orientation. Clear depiction of this venous pathway matters when you are teaching anterior chest wall drainage and the connections between the thoracic wall and the central venous system. The internal thoracic vessels sit in the surgical corridor for internal mammary artery harvest in coronary artery bypass grafting, and venous bleeding from the accompanying internal thoracic veins is a familiar operative issue during dissection behind the costal cartilages. It is also a practical map for procedures that traverse the parasternal region, including chest tube placement or parasternal blocks, where staying lateral to the sternal border reduces risk to the internal thoracic neurovascular bundle. Small vessel, big consequence.
