Description
Arising from the lesser horn and adjacent body of the hyoid bone, the chondroglossus runs superiorly and anteriorly to blend with the intrinsic musculature along the posteroinferior tongue, immediately medial to the hyoglossus in a true sagittal section. Superior to its origin sit the tongue base and vallecular region, while the laryngeal inlet and epiglottis lie posteroinferiorly, separated by the hypopharyngeal airway. Bony landmarks of the male head and neck include the cranial vault, maxilla and mandible with the mouth held open, and the upper cervical vertebrae aligned posterior to the pharynx. Because the chondroglossus is often treated as a deep fascicle of hyoglossus, a midline or paramedian sagittal cut clarifies its attachment to the hyoid and its fiber direction into the tongue base, which is hard to appreciate in surface views. This anatomy matters during transoral and suprahyoid approaches, where dissection near the hyoid and tongue root risks injury to the hypoglossal nerve and lingual artery that course in the same compartmental plane as the extrinsic tongue muscles.
