Description
Seen in lateral sagittal section, the mylohyoid forms a muscular sling between the medial surface of the mandibular body (mylohyoid line) anteriorly and the hyoid bone inferiorly, meeting its fellow at the midline raphe to create the floor of the oral cavity. Superior to its fascicles lie the tongue and oral mucosa, while inferior to it sit the anterior belly of the digastric and the submental region. Posteriorly, the free edge of mylohyoid relates to the hyoglossus and the upper pharynx, with the hyoid acting as the shared anchor for suprahyoid and infrahyoid groups. Adjacent context in this section logically includes the mandibular teeth and alveolar process, the maxilla and nasal cavity, cervical vertebrae, and strap and masticatory muscles such as sternocleidomastoid, masseter, and temporalis. Functionally, the mylohyoid elevates the hyoid and floor of mouth during the oral and pharyngeal phases of swallowing and helps stabilize the mandible during early chewing. This view clarifies why infections track the way they do: odontogenic infection from mandibular molars can dissect above the mylohyoid into the sublingual space or below it into the submandibular space, a distinction that guides drainage in deep neck infection and Ludwig angina. Small muscle, big consequence. The relationship to the hyoid also frames airway mechanics during sleep and during laryngoscopy positioning, where suprahyoid tone influences tongue base and epiglottic displacement.
