Description

Arising from the posterior abdominal cavity, the colic lymphatic vessels course along the mesocolon toward chains of epicolic and paracolic nodes at the bowel wall and along the marginal arcade, then converge on intermediate nodes clustered around the ileocolic, right colic, and middle colic arteries. Superiorly and medially, these channels drain to the superior mesenteric lymph nodes adjacent to the superior mesenteric artery, with continuity toward preaortic nodes near the abdominal aorta. Retroperitoneal landmarks frame the pathways: the kidneys lie posterolaterally, the abdominal aorta and inferior vena cava run vertically in the midline, and the psoas major and iliacus (iliopsoas) form muscular borders on either side. A posterior perspective matters because it aligns the lymphatic drainage of the colon with the visceral arterial origins that guide both oncologic staging and surgical planes. During complete mesocolic excision with central vascular ligation for right-sided colon cancer, nodal yield depends on following lymphatics to their root at the superior mesenteric vessels, while avoiding injury to the superior mesenteric vein and nearby autonomic fibers from the superior mesenteric plexus.
