Description

Centered in the transverse spinal cord section, the butterfly-shaped gray matter forms prominent posterior (dorsal) horns and broader anterior (ventral) horns, surrounded circumferentially by white matter columns. Along the posterolateral and anterolateral sulci, multiple dorsal and ventral rootlets converge into dorsal (sensory) and ventral (motor) nerve roots as they traverse the subarachnoid space. Posteriorly, the dorsal columns lie medial to the dorsal horns, while the lateral and anterior funiculi wrap laterally and ventrally around the cord. Orientation is clear. Seeing the dorsal and ventral roots in the same cross-section matters when you teach, diagnose, or operate around segmental cord anatomy. Dorsal root fibers enter the dorsal horn and are associated with sensory pathways, a relationship that explains radicular pain and dermatomal sensory loss from dorsal root or dorsal root ganglion involvement, such as a posterolateral disc herniation compressing the traversing root. Ventral root efferents arise from anterior horn motor neurons, so selective ventral root compromise or anterior horn pathology (for example, poliomyelitis or motor neuron disease) maps cleanly to lower motor neuron weakness and atrophy without primary sensory loss.
