Description
Arising from the anterior ramus of C5 (often with a C4 contribution), the dorsal scapular nerve tracks posteroinferolaterally from the lower cervical spine toward the medial border of the scapula. In the posterior triangle it passes deep to the levator scapulae, then continues inferiorly between or deep to the rhomboid muscles as it approaches the scapular spine. Nearby relationships you would expect in this head and upper torso model include the cervical vertebrae and spinal nerve roots, the clavicle and scapula, and adjacent components of the brachial plexus situated more anterolaterally. Course matters here. The dorsal scapular nerve is a frequent point of confusion with the long thoracic and spinal accessory nerves during neck and shoulder teaching because all three can present with periscapular pain and abnormal scapular motion, yet they supply different muscles and produce different patterns of winging. This view emphasizes its deep relationship to levator scapulae and its trajectory toward rhomboid minor and major, a pattern that aligns with clinical dorsal scapular neuropathy from traction, repetitive overhead activity, or entrapment as the nerve pierces the middle scalene, producing medial scapular border pain and weakness of scapular retraction and downward rotation.
