Arising from the anterior rami of C3 and C4, the supraclavicular nerve appears at the posterior border of the sternocleidomastoid within the superficial cervical plexus and then descends inferiorly toward the clavicle in three named groups. Medial, intermediate, and lateral supraclavicular branches fan out over the anterolateral neck and upper thorax, crossing superficial to the clavicle and tracking toward the sternoclavicular region, midclavicular area, and acromial region respectively. Bony landmarks including the clavicles, scapulae, upper ribs, and portions of the cervical spine orient the nerve’s course relative to the shoulder girdle. Cranial and upper cervical nerve pathways around the face and neck provide adjacent context, but the supraclavicular branches remain the focus in the lower lateral cervical and supraclavicular fossa region. For clinicians, the supraclavicular nerve matters because it carries cutaneous sensation from the clavicle, superior pectoral region, and shoulder cap, territories that become conspicuous in postoperative numbness after open reduction and internal fixation of clavicle fractures. This view also supports safe planning for supraclavicular and infraclavicular incisions, where superficial nerve branches are vulnerable as they traverse the platysma and superficial fascia and pass across the clavicle.
