Description
Arising from cervical spinal nerves, the ansa cervicalis forms a distinct loop on the anterior surface of the carotid sheath, typically superficial to the common carotid artery and internal jugular vein and deep to the infrahyoid strap muscles. A superior root descends with the hypoglossal nerve (CN XII) from C1 fibers, while the inferior root ascends from C2 to C3, meeting anteriorly in the upper neck. Surrounding landmarks in a male skeletal overlay include the mandible and hyoid region superiorly, the cervical vertebrae posteriorly, and the clavicles and superior thoracic cage inferiorly. Fine peripheral nerve trajectories also extend across the cranial region, consistent with cranial nerve pathways exiting the skull base. Surgical anatomy of the ansa cervicalis matters because it sits in the operative corridor of carotid endarterectomy, thyroidectomy, and tracheostomy, where dissection along the carotid sheath and strap muscles can place the loop and its branches at risk. Injury or entrapment affects motor supply to omohyoid, sternohyoid, and sternothyroid, leading to impaired laryngeal elevation and dysphagia, and it can be mistaken for hypoglossal dysfunction if the C1 component is not understood. This view also clarifies why the ansa cervicalis is harvested for reinnervation procedures, such as ansa to recurrent laryngeal nerve anastomosis in unilateral vocal fold paralysis, given its accessible location and predominantly motor fibers.
