Description
Arcing across the anterior cervical triangle, the ansa cervicalis forms a nerve loop superficial to the carotid sheath and deep to the sternocleidomastoid, with its superior root hitchhiking briefly with the hypoglossal nerve (CN XII) before descending. Inferiorly, fibers from the cervical plexus (typically C2 to C3, sometimes with C4 contribution) rise to meet the superior root, creating the characteristic junction and curvature emphasized in the artwork. Nearby, the clavicle and superior scapula establish the cervicoscapular framework, while strap muscles in the infrahyoid compartment sit medial and anterior to the loop. Small accompanying vessels course in parallel, reinforcing the close packing of neurovascular structures in this region. Surgical orientation depends on seeing this relationship correctly. During carotid endarterectomy, selective neck dissection, or submandibular approaches, the ansa cervicalis and the hypoglossal nerve lie in the same operative field, and confusing the superior root for CN XII can lead to avoidable traction or ligation. The clinical payoff is functional: the ansa supplies motor innervation to sternohyoid, sternothyroid, and omohyoid, so injury can contribute to dysphonia or impaired laryngeal elevation, and the loop itself is routinely harvested for ansa cervicalis to recurrent laryngeal nerve reinnervation in unilateral vocal fold paralysis.
