Description
Running in the anterior cervical triangle, the ansa cervicalis forms a nerve loop superficial to the carotid sheath and typically lies on, or just anterior to, the internal jugular vein, with its superior root (descendens hypoglossi, mainly C1 fibers traveling with cranial nerve XII) descending from the submandibular region to meet the inferior root (descendens cervicalis, C2 to C3). From that loop, branches course medially toward the infrahyoid strap muscles, crossing the anterior surface of the carotid vessels on their way to the sternohyoid, sternothyroid, and omohyoid. Superiorly, the mandible and hyoid region frame the field; inferiorly, the clavicles and superior thoracic aperture provide bony context. An anterior perspective matters because it matches the operative corridor used in carotid endarterectomy, thyroidectomy exposure, and tracheostomy, where the ansa cervicalis can be encountered and preserved or intentionally harvested. This is the loop surgeons use for laryngeal reinnervation, most commonly an ansa cervicalis to recurrent laryngeal nerve anastomosis after iatrogenic RLN injury from thyroid surgery. Small nerve, big consequences. Use it in head and neck anatomy teaching to anchor relationships between the cervical plexus, hypoglossal nerve, and carotid sheath, and in surgical atlases to illustrate safe planes during anterior neck dissection and strap muscle management. The combination of skeletal landmarks and highlighted cervical nerves also supports radiology and ultrasound teaching when correlating surface anatomy with deeper neurovascular structures.
