Description
Running anteriorly across the root of the neck, the cords and proximal branches of the brachial plexus form a compact nerve mesh around the subclavian artery as it curves laterally toward the axilla. Superior to the clavicle, the nerve elements lie in the interscalene interval between the anterior and middle scalene muscles, then descend inferolaterally toward the costoclavicular space. The clavicle and adjacent shoulder girdle landmarks frame the plexus superficially, while the subclavian vein sits more anterior and inferior to the artery, separated in part by the anterior scalene. Relationships are tight. This anterior perspective matters because it mirrors the corridor used in supraclavicular exposure and ultrasound-guided regional anesthesia, where needle trajectory must account for the artery, vein, pleura, and the clustered neural targets. Brachial plexus blocks at the level of the trunks and divisions depend on recognizing how the plexus wraps around the subclavian artery, and why vascular puncture or pneumothorax remains a procedure-specific risk. The same anatomy underlies thoracic outlet syndrome, in which neural compression can occur between the scalene muscles or beneath the clavicle, producing pain, paresthesia, and weakness in the upper limb distribution. Use this illustration in gross anatomy and regional anatomy courses when teaching the root of neck and transition to the axilla, or in anesthesia and pain medicine materials explaining the supraclavicular block and common complications.
