Description
Seen from a superior perspective, the cystic vein is traced as a small venous channel draining the gallbladder bed and neck region, coursing medially toward the porta hepatis. Its termination is shown joining the portal venous system either directly into the portal vein or indirectly through small hepatic venous tributaries within the hepatocystic triangle. Proximally it lies adjacent to the cystic duct and cystic artery, and it sits inferior to the undersurface of the right lobe of the liver where the gallbladder fossa would be encountered. Small vessel. Real consequence. Surgical anatomy drives the value of this view. During laparoscopic cholecystectomy, the cystic vein is a frequent source of troublesome bleeding when the gallbladder is dissected off the liver or when traction opens short venous channels into the portal territory, and its variability means it is not reliably controlled by clipping the cystic artery alone. The superior orientation also reinforces the relationship of the venous drainage to the portal triad and clarifies why bleeding here can be brisk and obscuring even though the vessel itself is small.
