Description
Running vertically along the posterior abdominal wall, the ascending lumbar vein tracks on each side of the lumbar vertebral bodies, positioned posterolateral to the psoas major and medial to the quadratus lumborum. Segmental lumbar veins join it along its course, while nearby spinal nerves exit the intervertebral foramina and pass laterally across the muscular plane. Superiorly, the vessel approaches the thoracolumbar junction where it continues toward the azygos system on the right and the hemiazygos or accessory hemiazygos system on the left. The pelvis and lower ribs frame the field. Clinically, this vessel matters because it forms a major longitudinal collateral channel between the common iliac veins and the azygos system, a pathway that becomes prominent in inferior vena cava obstruction or thrombosis. Surgeons and interventionalists also encounter the ascending lumbar vein as a frequent source of bleeding during retroperitoneal exposure of the lumbar spine and when mobilizing the psoas in lateral transpsoas approaches. Its communication with the epidural venous plexus helps explain routes of hematogenous spread and the potential for venous congestion around the spinal canal. Use this posterior perspective for teaching posterior abdominal wall anatomy in gross anatomy and neurosurgical anatomy courses, and for illustrating collateral venous return in vascular surgery, radiology, or case reports on IVC occlusion.
