Beneath the cheek skin, the deep masseter occupies the lateral surface of the mandibular ramus, its fibers running predominantly superior to inferior as they descend toward the angle of the mandible. Anteriorly, its muscular belly sits just posterior to the oral commissure region and inferior to the zygomatic arch, separated superficially from the dermis by the superficial masseter and the parotidomasseteric fascia. Medially, the ramus provides the bony substrate, while superiorly the muscle approaches the zygomaticomaxillary region where the masseteric origin is anchored. The left and right masseteric contours frame the lower face laterally. Anterior skin-on context matters for the masseter because it is a palpable, clinically targeted elevator of the mandible, and hypertrophy or spasm often presents as a broadening of the gonial angle with jaw pain that patients localize to the cheek. Botulinum toxin injection planning, management of bruxism-related myalgia, and evaluation of masseteric enlargement all depend on understanding how the deep layer sits relative to the superficial layer, the parotid gland, and the course of the facial nerve branches that cross the masseter en route to the mimetic muscles. This is a high-risk neighborhood.
