Description
Radiating from a centrally placed terminal hepatic venule (central vein), plates of hepatocytes form a polygonal lobule with intervening hepatic sinusoids running centrifugally to centripetally toward the venous outflow. At the lobular periphery, portal tracts sit at the corners, where a branch of the portal vein and a hepatic arteriole deliver mixed blood into the sinusoidal network while a bile ductule occupies the same connective tissue space. Sinusoids course between adjacent hepatocyte cords, their lumen lined by fenestrated sinusoidal endothelial cells with Kupffer cells along the wall, and separated from hepatocytes by the space of Disse. Directionality is the teaching point. Blood flows from the portal triads toward the central vein, but bile flows in the opposite direction through bile canaliculi toward the bile ductules, a relationship that underpins classic exam questions and real diagnostic patterns. Zonal physiology becomes legible in this arrangement: periportal (zone 1) hepatocytes see the highest oxygen and nutrient delivery, while centrilobular (zone 3) hepatocytes sit closest to the central vein and are the first to suffer in ischemic injury and acetaminophen toxicity; the same zone is also where CYP450 activity concentrates. Pathology reads along the same map, from pericellular “chicken wire” fibrosis in steatohepatitis to sinusoidal obstruction syndrome with painful hepatomegaly and congestion centered on the terminal venules. Use this plate for histology and pathology teaching in medical, dental, and biomedical curricula, and for figure support in hepatology manuscripts discussing portal inflow, sinusoidal exchange, and patterns of necrosis or fibrosis.
