Description

Radiating hepatic plates of hepatocytes form wedge-like sectors around a central vein, with blood sinusoids coursing between the plates toward the lobular center. At each lobular corner, a portal triad anchors the periportal region, pairing a hepatic artery branch with a portal venule and an interlobular bile duct within connective tissue. Arterial twigs track medially from the portal tract into the sinusoids, running alongside the portal venous inflow but remaining distinct from the bile canaliculi, which stream in the opposite direction toward the bile duct. Orientation is schematic but anatomically faithful. Arterial anatomy at the lobular scale matters because the hepatic artery is the oxygen-rich component of the dual blood supply, and its mixing with portal venous blood defines the steep physiologic gradient from zone 1 (periportal) to zone 3 (centrilobular). That gradient predicts injury patterns: centrilobular hepatocytes sit farthest from the arterial inflow and are the first to suffer in hypoxic states such as shock liver, while periportal regions bear the brunt of cholestatic and biliary pathology. For transplant, hepatic artery thrombosis threatens the bile ducts first, since the peribiliary plexus depends heavily on arterial flow. Small vessel placement becomes a teaching point, not decoration.
