Description
Dorsal cochlear nucleus anatomy is centered at the dorsolateral pontomedullary junction, where the rostral medulla meets the caudal pons along the floor and lateral recess of the fourth ventricle. In this posterior view, the nucleus sits lateral to the midline dorsal columns and medial to the cerebellar attachment zone, with the inferior cerebellar peduncle (restiform body) forming a prominent lateral neighbor. The cochlear nerve (CN VIII) root entry region lies at the cerebellopontine angle anterolateral to the nucleus, while the obex and caudal fourth ventricle define the inferior and medial dorsal landmarks. Layering within the dorsal cochlear nucleus is emphasized as a stratified sheet curving over the dorsolateral brainstem surface. Small but exact. Clinically, the dorsal cochlear nucleus is a frequent teaching site for tonotopy and early central processing in the auditory pathway, and it is implicated in tinnitus models where altered excitability and somatosensory-auditory integration are discussed. Lesions at the lateral pontomedullary region, including PICA territory infarcts (lateral medullary syndrome) or demyelinating plaques, can involve the cochlear nuclei and present with ipsilateral auditory symptoms that contrast with the predominantly bilateral projections of higher auditory relays. The fixed posterior perspective helps localize the nucleus relative to the fourth ventricle and inferior cerebellar peduncle, the same neighborhood navigated in posterior fossa and fourth ventricular corridor discussions.
