Description
Dorsal nucleus of the lateral lemniscus occupies the dorsolateral pontine tegmentum, positioned superior to the superior olivary complex and medial to the lateral surface of the upper pons. From this rear (posterior) viewpoint, the nucleus is localized deep to the dorsal brainstem contour, adjacent to the course of the lateral lemniscus as it ascends toward the inferior colliculus of the midbrain. Nearby landmarks in the same region include the floor of the fourth ventricle medially and the superior cerebellar peduncle more laterally at the pontomesencephalic junction. Small, but specific. Functionally, the dorsal nucleus of the lateral lemniscus forms part of the central auditory pathway, contributing to binaural processing and temporal features of sound before relay to the inferior colliculus; lesions in this dorsolateral pontine territory can alter auditory brainstem responses (ABR) and contribute to central hearing deficits that do not match a peripheral cochlear pattern. The posterior orientation is useful when correlating with dorsal pontine pathology such as demyelinating plaques, intrinsic pontine glioma extension, or a dorsolateral pontine infarct in the distribution of perforators from the basilar and anterior inferior cerebellar artery systems, where neighboring long tracts and auditory relays sit in tight proximity. Depth cues and separation of the nucleus from surrounding tegmentum help you teach why a small brainstem lesion can yield disproportionate neuro-otologic findings.
