Description
Dorsal aspects of the male hand and fingers are presented with emphasis on the extensor expansion (dorsal digital expansion) over the metacarpophalangeal joints and proximal phalanges. The extensor digitorum tendons course distally across the dorsum of the metacarpals, then broaden into a thin aponeurotic hood that sits posterior to the metacarpal heads and wraps around each MCP joint. From this expansion, a central slip continues to the base of the middle phalanx while paired lateral bands track distally along the posterolateral margins toward the distal phalanx, closely related to the underlying proximal interphalangeal and distal interphalangeal joints. Bony landmarks of the carpals, metacarpals, and phalanges anchor the mechanism. Extensor expansion anatomy is where tendon mechanics become clinical. Disruption of the central slip at the PIP joint, whether from a laceration or a jammed finger injury, can permit volar migration of the lateral bands and progression to a boutonniere deformity, often missed early when swelling masks loss of active PIP extension. Sagittal band injury at the MCP joint can also allow extensor tendon subluxation, producing snapping during finger flexion and extension. Small structures. Big consequences. Hand surgery texts and operative consent materials use this posterior perspective to clarify zone IV and zone V extensor tendon injuries and the rationale for splinting positions that protect the central slip or sagittal bands. Anatomy and kinesiology courses can pair the image with demonstrations of intrinsic muscle contributions (lumbricals and interossei) to the lateral bands to explain why MCP flexion can coexist with IP extension.
