Description
Seen from the posterior aspect of a male hand and wrist, the extensor digitorum tendons course distally across the dorsum of the carpus beneath a synovial tendon sheath and peritendon, then fan toward the index through little fingers over the metacarpals and metacarpophalangeal joints. Proximally, the sheath aligns with the dorsal carpal ligamentous roof of the extensor retinaculum region, forming a low-friction tunnel where the tendons sit superficial to the carpal bones and dorsal intercarpal ligaments. Distal to the wrist, the tendons lie dorsally to the metacarpal shafts and phalanges, maintaining a midline position over each digit before blending into the extensor expansion.
