Description
Across the dorsum of the male foot, the extensor expansions (extensor hoods) spread over the metatarsophalangeal joints and proximal phalanges, blending with the tendons of extensor digitorum longus to toes II through V and extensor hallucis longus to the hallux. Each hood sits superficial to the phalanges and caps the MTP joint, then continues distally as a dorsal aponeurosis that organizes into a central slip toward the base of the middle phalanx and terminal bands toward the distal phalanx. Proximally, these tendinous expansions course anterior to the ankle joint over the tarsus, then fan out distally over the metatarsal heads. Lateral contributions from extensor digitorum brevis are implied as they join the long extensor tendons near the dorsal midfoot. Functionally, the extensor hood is where toe extension is distributed across multiple joints: the long extensor tendons extend the MTP joints directly and, via the dorsal aponeurosis, transmit force to extend the interphalangeal joints. Small structure, big consequence. This dorsal apparatus matters in evaluation of claw toe and hammer toe deformities, where imbalance between extensor tendons and the intrinsic muscles, plus attenuation of the hood and plantar plate pathology at the MTP joint, drives hyperextension proximally with flexion distally. It also frames dorsal surgical approaches to the toes, where careful handling of the extensor expansion affects postoperative extension lag and adhesions. Podiatry and orthopaedic teaching sessions on forefoot biomechanics will find this view useful when explaining how extensor hallucis longus and extensor digitorum longus act through an aponeurotic expansion rather than inserting as a simple tendon.
