Description
Dorsally oriented at the wrist, the extensor retinaculum forms a transverse fibrous band spanning from the distal radius laterally toward the ulnar styloid and triquetrum medially, roofing the extensor compartments as the long extensor tendons pass from forearm to hand. Immediately deep to it, the tendons of extensor digitorum, extensor indicis, and extensor digiti minimi run distally toward the metacarpophalangeal joints, while extensor carpi radialis longus and brevis lie more lateral and extensor carpi ulnaris tracks along the ulnar border. Proximally, extensor muscle bellies transition into gleaming tendons; distally, the tendons fan toward the dorsal digital expansions over the metacarpals and phalanges. Functionally, the retinaculum prevents bowstringing during wrist and finger extension by maintaining tendon apposition to the dorsal carpus, and it defines the six dorsal extensor compartments used as practical surface anatomy during examination and injection. Stenosing tenosynovitis is compartment-specific, most famously in the first dorsal compartment where abductor pollicis longus and extensor pollicis brevis produce de Quervain disease, while intersection syndrome presents more proximally where the first compartment tendons cross those of extensor carpi radialis longus and brevis. Surgeons also reference these relationships during dorsal wrist arthroscopy, tendon transfers, and release procedures when protecting the posterior interosseous nerve and minimizing extensor tendon adhesions.
