Description
Posteriorly, the fascia brachii forms a dense sleeve around the upper arm, blending proximally with the deep fascia over the posterior deltoid and continuing distally toward the olecranon region where it becomes continuous with the antebrachial fascia. Beneath this aponeurotic investment, the triceps brachii (long head medially, lateral head laterally, and the deeper medial head) fills the posterior compartment, with the triceps tendon converging inferiorly toward the ulna. Medial and lateral intermuscular septa extend from the fascia to the humerus, separating triceps posteriorly from brachialis and biceps brachii anteriorly, while the posterior axillary fold is outlined superiorly by latissimus dorsi and teres major. Fascial compartmentalization in the brachium matters because it defines surgical planes and the pathways of neurovascular structures in the arm. The radial nerve and profunda brachii artery course in the radial (spiral) groove of the humerus deep to the lateral head of triceps, a relationship that explains wrist drop after midshaft humeral fractures and guides posterior approaches for humeral fixation. Tightening within these fascial boundaries also frames the clinical logic of acute compartment syndrome of the arm after trauma, when rising intracompartmental pressure compromises perfusion. Use this posterior male view to teach upper-limb deep fascia in gross anatomy lab, to illustrate compartment anatomy in orthopedic or trauma texts, or to support operative notes describing posterior humeral exposure and triceps-splitting intervals.
