Description
Arcing within the medial hemisphere, the fornix is rendered in lateral profile as a compact, C-shaped white matter tract coursing superior to the thalamus and beneath the corpus callosum. Posteriorly it arises from the hippocampal formation as the fimbria and crura of the fornix, then sweeps anteriorly into the body of the fornix before descending as the columns toward the hypothalamus and mammillary bodies. The thalamus sits inferior and slightly posterior to the body of the fornix, while the amygdala lies anteromedial to the hippocampal head in the medial temporal lobe. Brainstem and cerebellum provide posterior fossa orientation, anchoring the limbic structures in standard neuroanatomical topography. A lateral view clarifies relationships that get lost in purely midsagittal diagrams, namely how the fornix bridges medial temporal lobe memory circuitry with diencephalic targets. Injury to this tract, whether from colloid cyst obstruction with hydrocephalus, tumor resection near the foramen of Monro, or anterior commissure and basal forebrain approaches, can produce disproportionate anterograde amnesia. This is the Papez circuit made tangible.
