Description
Seen from a posterior perspective, the fornix arches as paired white-matter bundles along the medial aspect of the cerebral hemispheres, closely related to the hippocampal formation and the undersurface of the corpus callosum. Its crura sweep posteriorly from the hippocampal fimbriae and converge toward the midline to form the body of the fornix, while the columns course anteroinferiorly toward the mammillary bodies. Posteriorly, the fornix sits just inferior to the splenium of the corpus callosum and adjacent to the hippocampal commissure, with the septum pellucidum typically interposed superiorly between the fornix and callosal body. Deep midline anatomy, cleanly organized. This posterior emphasis matters when you need to teach or plan around limbic disconnection and memory circuitry, because the fornix is the dominant efferent pathway of the hippocampus (Papez circuit) and injury predicts anterograde amnesia. In neurosurgical corridors near the third ventricle (for example, endoscopic approaches to colloid cysts at the foramen of Monro), fornical columns are at risk from traction, cautery, or transventricular instrument passage. A posterior view also helps clarify commissural relationships that get conflated on axial MRI, separating the hippocampal commissure from the more dorsal splenial callosal fibers.
