Description
From a posterior perspective, the paired cerebral hemispheres frame the midline telencephalon, where the corpus callosum forms the dominant interhemispheric commissure. The splenium, the broad posterior terminal portion, lies superior to the tectal region of the midbrain and posterior to the thalamic mass, which is often color-coded to emphasize its diencephalic position. Inferiorly, the brainstem (midbrain, pons, and medulla oblongata) continues into the foramen magnum direction, while the cerebellar hemispheres and vermis sit posteroinferior to the cerebrum, separated by the tentorial plane. Lateral to the upper brainstem, cranial nerve rootlets may be suggested as they course toward their respective foramina. Posterior callosal anatomy matters when you need to localize disconnection syndromes and posterior circulation pathology. Lesions involving the splenium, including ischemia in the posterior cerebral artery territory or transient splenial lesions seen with seizures and antiepileptic drug changes, classically disrupt interhemispheric transfer of visual information and can produce alexia without agraphia when the left occipital cortex is isolated from language areas. This angle also clarifies the close relationship of the posterior corpus callosum to the thalamus and midbrain, a relationship that influences the surgical corridor for pineal region and posterior third ventricular approaches. Neuroanatomy and neuroscience courses use this posterior view to teach telencephalic commissures in context, contrasting the corpus callosum with deeper diencephalic landmarks like the thalamus and the dorsal brainstem.
