Description
Arcing along the medial aspect of a sagittally sectioned cerebrum, the corpus callosum is highlighted as a broad commissural band spanning between the cerebral hemispheres, with its genu curving anteriorly and the splenium tapering posteriorly toward the occipital region. Superiorly it underlies the cingulate gyrus and lies deep to the cerebral cortex, while inferiorly it forms the roof of the lateral ventricle and relates to the septum pellucidum and fornix in the midline. The third ventricle sits inferior and medial to the callosal body, bordered laterally by the thalamus, with the hypothalamus positioned anteroinferior to the thalamus and continuous with the infundibulum leading to the pituitary gland. Posterior and inferior, the cerebellum occupies the posterior cranial fossa, and the brainstem (midbrain through medulla) descends centrally to meet the spinal cord. A lateral perspective on a mid-sagittal section is the clearest way to teach callosal topography and its relationships to the ventricular system. Clinically, the corpus callosum is a key landmark in MRI when assessing agenesis or dysgenesis, where an absent callosal arch accompanies colpocephaly and altered orientation of the ventricles, and in traumatic shear injury where callosal lesions correlate with impaired interhemispheric transfer. This anatomy also guides commissurotomy planning and helps localize deep midline masses that distort the third ventricle.
